MRI using a hepatocyte-specific contrast agent improves the diagnosis of choledochal cyst: Case report

Magnetic resonance imaging (MRI) contrast agents containing Gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DTPA) are liver specific agents that account for 50% of the hepatobiliary system. In this study, we present a case of choledochal cyst that was evaluated with gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DTPA) - enhanced magnetic resonance cholangiography (MRCP) imaging. On abdominal CT, a cystic mass was noted in the head of pancreas, suggesting the possibility of mainly a pseudocyst or a choledochal cyst. MRCP was performed for further evaluation; however on MRCP, the relation of the cyst with the biliary tract could not be proved. On the hepatobiliary phase of Gd-EOB-DTPA – enhanced MRI, the contrast in the biliary tract extravasated into the lumen of the cyst and the diagnosis of choledochal cyst was confirmed.
